Enrollment
- Once annual membership has been paid in full, the annual fee is NOT refundable
- Plan members are obligated to pay for all dental care services at time of service
- Membership is active and coverage begins the day the annual fee is paid in full, effective for 1 year
- Auto renewal is eligible
- Member will place credit card on file
Exclusions & Limitations
- This is not insurance
- Discount plan DOES NOT coordinate with other medical or dental insurances
- Services for injuries covered by workman’s compensation
